Person Centred Care

All care plans are developed with the wellbeing of the client being the foremost concern. No care is delivered without the consent of the client or the client’s family. At all times the standard and appropriateness of care is monitored and if changes are necessary they will be made in consultation with the client, the Care Operations Manager and the Primary Care Team in the HSE.

Regular care plan reviews are undertaken by our qualified Team Leaders and our Care Operations Manager to ensure our standard of care is always suitable and up-to-date.

Monitoring

Northside Home Care Services use a cloud-based, comprehensive scheduling/tele-monitoring system that works in tandem with a corresponding mobile app to ensure that no visits to clients are missed and that our carers arrive on time to them. Our carers are provided with a personal smart phone with the app preloaded. The app is also used by the carers to keep track of their schedules, create timesheets and communicate important relative information to the office staff. Using the information generated by the app, the system is able to provide live updates on carer progression, monitor the level of care provided to each client and aid in creating dynamic careplans.

Frequent client satisfaction surveys allow us to monitor any areas of our care that may need improvement or adjustment to suit specific client needs.

Palliative Care

For someone with a life-limiting illness we offer support to address the needs of the client and the family in their own home and where possible to enhance the quality of life for the client. While understanding that this can be a challenging time for clients and families we endeavour to provide strong physical and emotional support during this time. Using a holistic approach as part of a multi-disciplinary team the quality of life for a client can be enriched and enhanced. Respite care can also be offered to family members when caring for loved ones with a life-limiting illness.
